The Image
As I already mentioned, your image looks obviously composed to me. Not because of the smudging (that could have been done better too, though I don't know how exactly) but because the colours of the two monochromatic images are different hues; the gate uses a somewhat blueish grey, whereas the background is more sepia-tinted. If you use Colors > Levels or Colors > Curves you can see that R, G and B values aren't as proportional to another as they should be in true monochrome.
As to how to fix it: eh… my "I have no clue what I'm doing"-solution would be going Color > Desaturate > Color to Gray. But that's not pretty. I'm not particulary good with image manipulation (and this is no faux humbleness or some bs), so I refer you to the Captain.
Stylistic
Should have mentioned it before, but "Explosion" is not much on an anomaly in itself. (You call throwing dynamite around a martial art anomalous?)
You can use [[footnoteblock]] to force the footnotes to appear before the rating module.
Not sure if the quoteblocked note looks as well when it is compressed due to the image being to the side; I preferred the way it was in your sandbox.
"Item" is a proper noun in this case and belongs capitalized.
There are still SPaG errors (mostly S), in your classic homophonic confusion. E.g. "do to" should be "due to", etc.
We're usually not putting Item code names into double quotes.

Having parts of the thing accidentally contained by dropping them into the ocean (with water being very good at absorbing or deflecting electromagnetic waves) is a clever move.
The schism in your continuity is actually something that somehow comes as a surprise for me, but the the mention that it was "stolen back" implies either somebody is doing a bad job covering up their traces, or authorial oversight in wording.
Are we to lazy to search a mere 12600 square meters for the gate? Sure, its a tiny piece, but unless the Foundation got some slow-mo recordings of the explosions with the exact location, their "knowledge of its reappearance patterns" shouldn't help much in locating a needle in a haystack. Just giving up on the pieces seems dumb.
Interpretation
To me, this looks like the Foundation is somehow in charge of this thing. They were able to make somebody (re)appear together with the Item; consider how it originally was deemed sentient, but now isn't? Almost as if the pilot is now missing. The addition of "Demonic Passenger" makes me guess its one of the things from Meta's personal menagerie of abominations, if not himself. This is further corroborated by sigh mysterious blanking of records in 2016.
Eschew elucidation, espouse obfuscation.
