From how I understood it:
Superhuman experiment gone wrong, with previous knowledge that it would go wrong.

The text itself is pretty dense (as in much information in long sentences). I could also not determine if there is a plan to end the world at play.

I will have to make a full breakdown of this until I have extracted all its meaning.

What is "institutional memory" and why wouldn't it come out of a corpse factory?

The forgetting problem kept winnowing down until it was only us who could follow testing with the cooperative process.

Sounds like the Insurgency and its cross-testing, but that "forgetting problem" makes it difficult. Anderson Robotics, MC&D and the Wanderers Bookclub all do there own cross-testing.

We had nobody left to try on, so things fell apart.

If there is indeed Human Serum at play this could not happen.
